Feel free to have a spouse, carer and support person help you with completing this survey.

Your participation in this study is voluntary. The survey takes approximately 10 minutes. By completing this survey, it will be understood that you have consented to participate in the project, and that you consent to publication of the results of the project with the understanding that anonymity will be preserved.

You can withdraw from the survey at any point, without giving a reason, up until you click submit on the survey. At this point, your data will be anonymously merged with all other surveys and cannot be removed.

Your responses to the survey will remain confidential and will be not be identifiable. These will be converted to anonymous numbers in a secure data file. The data will be stored separately in an encrypted electronic backed up database in the Department of Speech Science.

Only the researchers will have access to your data. Your survey data will be stored for six years after the study. After which time, all data will be deleted.
